In the severest forms, the cleft extends through the alveolus and passes between the premaxillary bone and the rest of the maxilla. The anterior maxillary teeth are considerably larger than the premaxillary teeth. The morphology of the premaxillary fragments may be more helpful. In bilateral clefts, the central portion of the lip, the premaxillary segment, is not attached to the lateral portion of the lip on either side.
