In one patient, MRI showed the presence of increased fat in the epicardium. Transmural infarcts involve the whole thickness of myocardium from epicardium to endocardium. Covering the outer surface of the heart is the epicardium. Resection revealed an area of the myocardium uncovered by epicardium.
